Fertility Testing/Treatment 

Benefits for pregnancy assistance treatment, including but not limited to infertility treatment, are excluded under most Blue Cross and Blue Shield of Nebraska contracts. Diagnostic testing done to determine the diagnosis of infertility, treatment of polycystic ovary disease and treatment of endometriosis are not considered to be infertility treatments.
