Federal Employee Program (FEP) – Pharmacy

FEP follows FEP medical policies and requires prior approval (PA) regardless of where the drug/prescription is obtained. These medical policies are maintained by Caremark, the FEP pharmacy benefit administrator.

Note the following: 

• Drugs administered by a pharmacy that require PA must be submitted to Caremark.
• Drugs administered by a physician or facility that require PA must be submitted to Blue Cross and Blue Shield of Nebraska.
