Electronic Funds Transfer (EFT)

If you are interested in having your claim payments electronically deposited into your bank account, please submit a completed EFT Enrollment Form. To download an EFT form, visit the Find a Form page.

NOTE: If you are changing EFT from one bank or account to another, you will receive paper checks until the effective date of the new bank account.
