Ancillary Claim Submission (Billing) Guidelines

The Blue Cross and Blue Shield Association mandates Plan compliance with the handling and processing of the following ancillary claims:

• Independent Clinical Laboratory
• Durable Medical Equipment and Supplies  
• Specialty Pharmacy
