Bilateral Procedures

Blue Cross and Blue Shield of Nebraska requires providers to bill bilaterally on one line. 

• Bilateral surgery code billed on one line with modifier 50 with one unit  
  • DO NOT bill bilateral surgery code on two lines (one with a modifier 50 and one without)  
• Modifiers LT and RT should not be used for bilateral surgery codes – only use modifier 50  
  • Radiology/HME codes are still acceptable to use LT and RT 

Note: For Home Medical Equipment and Radiology codes, bill Left and Right with modifiers LT and RT.
